The Cochrane Tobacco Addiction Group, based at the University of Oxford, is widely considered the global gold standard for evaluating evidence on smoking cessation interventions. Unlike individual studies that can be cherry-picked, Cochrane reviews synthesize every relevant randomized controlled trial (RCT) that meets strict quality criteria, then grade the certainty of the evidence using a standardized system (GRADE approach).
The August 2026 update is part of the Cochrane Living Systematic Review on electronic cigarettes for smoking cessation — a continuously updated review that incorporates new evidence as it becomes available. This update included evidence current through January 2026, making it the most comprehensive assessment to date.
In plain terms: Cochrane reviews are the closest thing medicine has to a "final word." When they rate evidence as "high certainty," it means multiple high-quality studies converge on the same conclusion, and further research is very unlikely to change that conclusion. This is not one study finding — it is the synthesis of 91 studies.
